Appointment Process
Upon accepting your fellowship you must complete the Form Q and return this via fax (574/631-6717) to the Attention of Denise Wright along with an up to date Curriculum Vita.
Visiting Fellows who are not US citizens or permanent residents must also complete and return a J-1 Visa Information form and a copy of the photo and bio page of your passport.
All of this information must be received before the formal letter of appointment or the visa documentation can be issued by the Provost’s Office.
After the required documents have been received by Denise, a formal letter of offer will be sent to you by the Vice President and Associate Provost of the University. You must accept that offer in writing for your appointment to be formalized.
Visiting fellows from abroad will not receive a Certification of Eligibility for an Exchange Visitor Visa (DS2019) until after the appointment process has been formalized. Fellows from abroad normally must have a J-1 visa; accompanying family members are classified as J-2. If visitors intend to have family members accompany them during their stay, they must provide each family member’s name, birthplace, date of birth, and citizenship on the J-1 Visa Information form.
Doctoral candidates who hold F-1 visas should arrange with officials at their universities to obtain all the necessary paperwork for “practical training” status before their appointment can be formalized.

Housing
As an Institute visiting fellow you are given the option to lease a furnished on-campus faculty apartment administered by the Notre Dame Office of Business Operations.
In order to reserve an apartment you must complete the Application for Occupancy form. One month’s rent is required to reserve an apartment. This deposit will be applied to the first month’s rent. In the event of a cancellation, Business Operations requires a 30-day notice or the deposit, in whole or in part (prorated based on the days less than 30), will be forfeited. Apartments will be assigned on a first come first served basis.
Six one-bedroom and four two-bedroom units with living rooms, bathrooms, and kitchenettes are available. Two-bedroom units are reserved for members who come with families. One single-bedroom unit is equipped to serve physically handicapped visitors. A one-bedroom apartment rents for $900 per month and a two-bedroom apartment for $1,125 per month (prices as of 2009) Please note that the above are estimated costs. As part of the visiting fellowship, the Institute subsidizes $250 of the monthly rent. There is no charge for cable television.
Apartment furnishings include pillows, bed linens, blankets, bedspreads, towels, dishes, cookware, and utensils. Each apartment is equipped with a television and kitchen appliances including a refrigerator, stove, oven, and microwave; some apartments have a dishwasher. Laundry facilities are available in the building.
University policy for apartment occupancy states that only visiting faculty members and immediate families (spouse and/or children) may live in on-campus faculty apartments. Pets are not allowed. Guests or other non-immediate family members (in-laws, nieces/ nephews, etc.) may visit for brief stays only. Exceptions to this policy will be made only under extenuating circumstances and must be approved well in advance by the director of asset management, Office of Business Operations. Requests for exceptions due to special circumstances should be directed through Denise Wright.
