Speaker Request Form
Please complete the following application and we will do our best to match you to a speaker. Use a separate form for each specific topic requested. If you want the same presentation to be given twice in one day, indicate both times in the appropriate box and provide details in the comment box. Because of students’ class schedules, it will be more difficult to schedule two engagements in one day. Note the exclusion dates due to Spring and Easter breaks.
Speakers are available to educators in the South Bend/Mishawaka/PHM school district area. A minimum of two weeks advanced notice is required.
